310 - Englewood Program Counselor (PT)
Provides direct care to persons with developmental and physical disabilities. Implements behavior programs and other habilitation programs. Facilitate and promote community integration and safety.
ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:
Assures safety and wellbeing of Marrakech individuals while adhering to agency policies and procedures.
Assists the individuals being served to coordinate their daily routine including food shopping, meal preparation, recreational activities, bills, budgets, and household chores as needed.
Purchases groceries as well as supervise and assist in the preparation of food for individuals, following special prescribed diets and food consistency orders. Implement eating procedures as written.
Assists individuals in daily activities, including daily hygiene functions
Essential employee – assists in scheduling trained staff to fill in shifts for this position during scheduled time off due to the essential function of this position.
Assists the individual in contacting the day program/work if the individual is home ill.
Assists individuals in household management tasks including cleaning.
Implements and documents behavioral programs, teaching strategies, exercise programs and other team recommended programs and procedures.
Documents communication logs, programs, incident reports, abuse and neglect reports, safety reports, fire drills, time exception reports, mileage logs, Therapp and other required documentation
Uses the timeclock or Sandata system when arriving/leaving shift
Lifts and Transfers individuals as required. Repositions individuals as stipulated in service plan.
(If DDS Medication Certified) Administers prescribed oral and topical medications under written direction of Physician. (Must become medication certified within allotted timeframe)
Follows the Critical Information for the program
Uses and maintains adaptive equipment (such as Century Tub, wheelchairs, Hoyer lift) according to safety guidelines.
Implements Physical/Psychological Management Training, CPR and First Aid
Performs housekeeping tasks
GENERAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:
Maintains barrier free exits at assigned program site
Maintains a safe and clean work environment
Stays awake on shift
Follows emergency on call procedures and all other Marrakech policies and procedures
Provides meaningful community experiences to individuals
Satisfies all training requirements per regulations, maintains all required certifications and licenses.
Assists in the completion of individual assessments
Asks for assistance from co-workers and supervisors when unsure of a job duty and refers to program policies for job clarification
Reports to work at alternative program site for shift work or for medication administration when assigned
Provides active treatment, incidental teaching and formal programming to individuals
Operates motor vehicles safely and in accordance with State Law (agency and personal automobiles) while on work related duties.
Reports unsafe conditions and situations immediately per agency policy and procedures
Represents the values, mission and vision of the organization
Models positive adult roles in the community
Fosters community belonging through individual person-centered appropriate activities.
Serves all customers with alacrity
Attends staff meetings as scheduled
Communicates with co-workers and other providers accurately and professionally
